Post Acute Care is available for short-term skilled nursing and therapy management between hospital and home. This focus centers on post acute rehabilitation utilizing professional physical, occupational, speech and aquatic therapists for strengthening and endurance to improve performance with functional activities. Rehabilitation can be extended once the patient returns home through our outpatient or in-home therapy programs. Skilled nursing care provides ongoing assessment, wound care and personal assistance in managing health issues.
